This is not an online registration form. To register, complete the form and then print it and mail or fax it to CLHS. Be sure to review the cancellation policy.

Information will appear on name badge. For additional group registrations (from same school only), attach a second sheet with full name of each participant.
3. Membership (optional): Discount rate will automatically be applied to participants purchasing a new membership.
CLMS Institutional Membership: $295 Includes member rates for all staff at all CLMS/CLHS/NHSA events, group discounts for the CLMS or CLHS/NHSA Annual Conference, newsletters and resources
CLMS Individual Membership: $60 Includes member rates for all CLMS/CLHS/NHSA events, newsletters and resources

Advanced registration required, extra fees apply, no refunds provided, sessions/equipment subject to change. These sessions are first-come, first-served and usually sell out. Registration Instructions
(Only purchase orders or credit card orders may be faxed.)
Regular Registrations - must be received with payment by mail or FAX in order to be processed.
On-Site Registration - after the "No Refund" date (see cancellation policy), you must register on-site (there is no additional cost). Payment by cash, check, purchase order, Visa or Mastercard will be accepted.
To ensure you have met strict payment deadlines, please:
Fax your credit card order or purchase order at least three days before the "No Refund" date (see cancellation policy), then call CLHS at (562) 430-3136 to confirm your fax has been received
If you must mail a check, please allow 10 days for mail delivery before the "No Refund" date (see cancellation policy). CLHS is not responsible for delayed mail.
To Cancel (except for ticketed focus sessions):
Cancellations must be in writing and faxed to (562) 430-5607. Please call to verify receipt of cancellations at (562) 430-3136.
